The word "Star" as it applies to "Porn Star" is used rather loosely when compared to actresses in movies where gymnastic sex isn't a mainstay, as pretty much anyone in an x-rated video gets the title.  But as far as true adult film legends go, Traci Lords is indeed one of the most notorious.  Certainly she's the great con-artist of the industry.  At 15 she ran away from home and went to LA where she made a fake ID and birth certificate.  Ridiculously well-developed for her age, she used her phony credentials to begin working in the porn industry. 

After appearing in nearly 100 videos between 1984-1986, it was revealed that Traci was underage, and they were all removed from the US Market.  All except her last film, made when she was 18, called "Traci I Love You", which became the only "legal" video available of the star, which became one of the greatest selling adult videos of all time, thanks to the new notoriety.  When I interviewed Ron Jeremy last year, he didn't have kind things to say about her, noting, "She really jeopardized a lot of people's livelihoods.  I mean, she looked 18 and had all the ID to prove it, and she kept her mouth shut about that until some folks were sued for making child pornography."
